Nursing homes & assisted living
Heart and vascular care for nursing home residents
A leg wound that will not heal, a cold foot, swollen legs, shortness of breath — residents of skilled nursing and assisted living facilities often need a cardiologist and a vascular specialist. amavita works with facilities across Miami-Dade to evaluate residents and coordinate their care with the facility team.

Who this is for
Facility teams
Directors of nursing, wound care nurses, social workers and administrators who need a heart or vascular evaluation for a resident.
Attending physicians
Physicians and nurse practitioners who follow residents and want a cardiology or vascular consultation.
Families
Sons, daughters and caregivers who are worried about a relative’s legs, feet or heart and want a specialist to take a look.
When to refer a resident
Ask for an evaluation when a resident has any of the following:
- A wound on the foot, heel or lower leg that is slow to heal or will not heal
- Leg or foot pain at rest, or pain with walking that eases when the resident stops
- A foot that is cold, pale, bluish or darker than the other
- Swollen legs, skin changes around the ankles, or a venous ulcer
- Shortness of breath, new swelling, or fast weight gain
- An irregular, very slow or very fast pulse
- Chest pain or pressure that comes back
Sudden severe leg pain with a cold, pale limb, chest pain that does not go away, or trouble breathing is an emergency — call 911.
How it works
How a referral works
-
01
Call us or leave a number
Call (305) 249-5666 or send the short form. Tell us the facility and who to call back. Please do not put the resident’s medical details in the online form — our coordinator will take them by phone.
-
02
We check the insurance
We accept Medicare and most Medicare Advantage and commercial plans, and we confirm the resident’s coverage before the visit.
-
03
The resident is evaluated
A focused exam and the non-invasive tests that are indicated, such as an ankle-brachial index (ABI), a vascular ultrasound, an electrocardiogram or an echocardiogram. Our coordinator arranges the time and place with the facility.
-
04
A clear plan for the care team
Findings and next steps are shared with the facility and the attending physician in plain language. Many residents are managed with medication and wound care.
-
05
A procedure only if it is needed
When a procedure is warranted, it is performed at amavita’s ambulatory surgery center at our Kendall office. It is an outpatient facility: the resident comes in, has the procedure, recovers, and returns the same day.
What we evaluate and treat
The problems we see most often in residents are circulation problems in the legs and long-standing heart conditions:
- Non-healing wounds and peripheral artery disease (PAD) — a leg circulation check shows whether enough blood is reaching the foot.
- Leg swelling and vein insufficiency.
- Heart failure, atrial fibrillation and coronary artery disease.
Procedures are performed at amavita’s dedicated ambulatory surgery center, co-located at our Kendall office — 9408 SW 87th Ave, Suite 303, Miami, FL 33176. An ambulatory surgery center is an outpatient facility: you come in, have the procedure, recover, and go home, without a hospital admission.
For cases requiring hospital admission, Dr. Martinez-Clark has attending privileges at HCA Florida Mercy Hospital and Jackson Memorial.
Consultations and follow-up visits are at either office: Kendall or North Miami Beach.
Medicare and most insurance plans accepted — we confirm your coverage before the visit. See the plans we accept.
