What decides your cost
With a plan from an employer, or one you buy yourself, what you pay depends on three things in your plan.
- Deductible. The amount you pay each year before the plan starts to pay.
- Copay. A set fee for a visit or test.
- Coinsurance. Your share of the bill, as a percentage, after the deductible.

If your Aetna plan is a Medicare Advantage plan
A Medicare Advantage plan replaces Original Medicare with a private plan. The plan, not Medicare, sets what you pay.
- Each plan sets its own copays. A copay is a set fee for a visit or test.
- Most HMO plans require a referral from your primary care doctor before you see a specialist.
- Some tests and procedures need prior authorization. That means the plan must approve them first.
- The 20% rule of Original Medicare does not apply. Your plan's own copays apply instead.
